Eine junge Frau namens Kris (Amy Seimetz) muss fürchterliche Qualen erdulden. Sie wird von einem Mann festgehalten und mit Gewalt dazu gezwungen Würmer zu essen, die jener zuvor gezüchtet hat. Durch die Einnahme der Würmer gerät sie in einen halluzinogenen Zustand, wird willenlos und verliert jeglichen Bezug zur Realität. Der Gärtner verlässt die Szene und ein Schweinezüchter tritt auf. Er experimentiert an der jungen Frau und nimmt merkwürdige operative Transfers zwischen ihr und einem Schwein vor. Durch einen Zufall trifft sie auf Jeff (Shane Carruth), der scheinbar das Gleiche wie sie durchstehen musste. Sie versuchen einander zu helfen, doch das Erlebte kann nicht einfach vergessen werden und so begeben sie sich auf die Suche nach dem Urheber ihres Unglücks und finden Unglaubliches heraus.
